Community Navigator Program

The Community Navigator is NBIS’s direct client services program.  It is a non-medical support service with a Registered Social Worker that helps people who have been affected by Acquired Brain Injury, both clients and caregivers.  The Navigator Program focuses on identifying and serving the needs of each individual client providing them with a personalized recovery plan based on their goals and linking them to both formal and informal resources in our community.

The Community Navigator Program helps NBIS clients by:

  • Answering client and caregiver questions and helping them navigate the health care system
  • Providing a non-medical client assessment and creating an individual action plan for the client
  • Assisting clients with self-management strategies to manage their ABI and caregiving roles
  • Linking people to resources and programs to assist with their recovery, including community, education and peer support programs
  • Providing ongoing case management, if required
  • Support with navigating housing applications
