Community Navigator Program
The Community Navigator Program is a personalized, client-centered program designed to support individuals impacted by Acquired Brain Injury (ABI), as well as their caregivers. This one-on-one, non-medical service provides compassionate care in identifying the unique needs, circumstances, and goals of each client.
The program helps clients to navigate and access a wide range of community resources, offering guidance and advocacy along the way. By focusing on empowerment and individualized care, the Community Navigator Program enables participants to more confidently navigate life after brain injury enhancing their overall well-being and quality of life.
The Community Navigator Program helps NBIS clients by:
- Addressing client and caregiver questions while guiding them through the complexities of various systems.
- Co-developing tailored action plans based on the needs of the client
- Assisting clients in implementing self-management strategies to navigate the challenges of ABI and caregiver roles
- Connecting individuals to community resources, such as financial assistance, rehabilitation services, employment services
- Offering ongoing case management as needed
- Providing support for applications and related processes (support in navigating complex systems)
